This return request is within 30 days from the delivery date

The original packing slip is included in the return package

A printed Return Merchandise Authorization (RMA) number is included (Issued after submission of this form)

The product is not fully consumed (if opened, must have at least 50% remaining)

For repeat purchases, the product is unopened and undamaged

By submitting this return request, I acknowledge that:

I have read and understood Advanced Lifespan’s 30-Day Money-Back Guarantee policy.
A 20% disposal fee applies to all opened products and a restocking fee applies to unopened products being returned.
Refunds will be issued via company check within 5 business days after inspection of the returned product.
If the return conditions are not met, the product will be disposed of at my expense, and no
refund will be issued.
Products received without an RMA number or the original packing slip will not be eligible for a
refund.
Refunds are not available for the “5 for 1” program or products purchased from third-party
vendors.

Once approved, you will receive a confirmation email with return shipping instructions.
Package the product securely and include: The RMA Number (Printed) The original packing slip
Ship via a trackable method with signature confirmation to the return address provided.
After the product is received and inspected, a refund will be processed within 5 business days.
